Unused parameter entities aren't errors. Nor, for that matter, are
multiply defined entities (only the first definition counts).

Some of these are used in the SGML DTD. Some are provided for
customization layers. Some are in calstblx.dtd which I'd prefer not to
touch.

The one in htmltblx.mod looks like it could be deleted, but is it
worth it?
