I wonder if this is worth doing. I can't see any practical way to do
it at the content model level. I can add a Schematron rule, of course.

But preventing

  <link xlink:href="..."><link xlink:href="...">foo</link></link>

won't prevent

  <citetitle xlink:href="..."><link xlink:href="...">foo</link></citetitle>

which amounts to the same thing. We've just said that it's undefined
what this means.
