DocBook V5.2 update

Hi folks,

Iâve opened four pull requests against the DocBook schema.

#242 just updates some documentation errors
#243 fixes the XInclude declarations, closing #239
#244 allows info as a top-level element, closing #241
#245 fixes the OASIS identifiers in catalog.xml

I propose that the TC should accept all of these changes.
We can then apply them, push a CR4 release, and ask OASIS
to publish that as the OASIS csd01 release.

Please accept my sincere apologies for the delay in producing these
changes.
