Released XSL DocBook Stylesheets version 0.13; udpated to XSL PR.

Released DocBk XML DTD version 3.1.6; mediaobject and
inlinemediaobject were missing from a number of content
models. Released "Simplified" DocBk XML DTD version 3.1.6.1;
unchanged, simply based on 3.1.6.

Released Modular DocBook Stylesheets version 1.46. Added CLASS
attribute to BODY tags; added citetitle to Bibliography
processing; small bug fixes.
