I personally use markup based on how the macro is *used* since the
programmer doesn't necessarily care that it is a cpp macro, if you've
defined it correctly, thus <constant>QM_NOT_FOUND</constant> which is
just a number <function>PMDM_PKT_CONTENT</function> which takes
arguments and is documented as a function elsewhere...
