Is what I am doing -- including a source code listing from a separate 
file -- so anathema to structured document authoring that years 
should go by with the only solutions being (a) a hack and (b) an 
external tool?  Why was such functionality eschewed from 
XML/XSL/DocBook?

(These are not meant to be rhetorical or facetious questions.  I'm a 
software developer, not a trained technical writer or structured 
documentation expert, and I seek to gain a better understanding of 
the school of thought that produced these technologies.)
