IMHO only:

- It doesn't belong in XML, it's an application issue
- It belongs in XSL, but it didn't make it into 1.0.
- We've always been hesitant to add it to DocBook because it appears
  that XLink would provide the same functionality.

With respect to my comment that <inlinegraphic format="linespecific">
is a hack, over time I've decided that my comment was a bit flip.
