I have. I have done the following:

* used db2pdf (with RH 6.2) which runs it at least twice
* use pdfjadetex by hand and ran it FIVE times

I still get no index references. Interestingly, when one of my
<indexterm>'ed words appears NEAR a figure with an id, and an xref
pointing to it, the page numbers are inserted correctly. Nonetheless,
I've tried small one page documents to index, and no matter whether I
give the indexterm an id or not.

This, for example, doesn't work:

<!DOCTYPE book PUBLIC "-//OASIS//DTD DocBook V4.1//EN" [
<!ENTITY gen SYSTEM "./genindex.sgml">


<title>Chaper One</title>
This is a <indexterm><primary id="test">test</primary></indexterm> test!


I have no idea what's up...
