This, as well, is a matter of stylesheet.  However, jade only
implements the simple-page-sequence (by contrast to full
page-sequence), which only allows a single header/footer setup per
simple-page-sequence.

With jade, you would need to define a new flow object, that rely on
the post-processor (eg. jadetex) to find the replacement text for each
page.
