Which set of style sheets are you using.  At one stage the DSSSL sheets had this problem if the top level element was not a recognised book division.

I have a <reference> section in a document, which contains a table within some of the <refsect1> sections. The problem is that each table is always called Table 1, whereas I think they should be Table I.1, Table I.2 etc. Is this a bug, or am I being dense in some way?
