You could add a role="comment" to your special paragraph in your step. In HTML output, you could just turn on the 'para.propagates.style' parameter which will add class="comment" to your output, and then use CSS to style it. For FO output, you can copy the match="para" template from block.xsl to your customization, and change the match to: match="step/para[@role='comment']" and then add properties to the paragraph's fo:block to indent and add grey. The comments are indented and displayed in grey to distinguish them from the action. Is there an element in DocBook that can do the same or should I just make an extra paragraph in the "step" and stop using the gray? How do you do it?
