Keeping Book Title and ToC together

Hi,
 
I'm building a set of books.

I'm trying to modify the fo stylesheet such that:
---------- page break ----------------
             Book Title
     Published 30 July 2004

Table of Contents 
... ...
---------- page break ----------------
Chapter 1. 
etc.


What I've got (so far) is this:
---------- page break ----------------
              Book Title
     Published 30 July 2004
---------- page break ----------------
Book Title
Published 30 July 2004
---------- page break ----------------
Table of Contents
... ...
---------- page break ----------------
Chapter 1.
etc.

Please, can someone point me in the right direction before I go nuts
trying to figure this out?  It is *so* frustrating!
