This is a bug in FOP. Here is what
the FO link looks like:
<fo:block>my entry,
<fo:basic-link
internal-destination="id428223">
<fo:page-number-citation ref-id="id428223"/>
</fo:basic-link>
</fo:block>
The link text is an empty
<fo:page-number-citation> element whose content is generated by the XSL-FO
engine. But FOP doesn't make it a link. If you put any actual text
inside the fo:basic-link element, it will form the link on the text you add, but
the page number is still dead.
