DocBook Publishers XSD

Hello world,

Here's a first attempt at a Publishers XSD. The publishers10.xsd file
and publishers11.xsd files differ only in the presence of xs:assert
elements in the 1.1 file, I think.

This schema presumably inherits all the bugs of the nascent DocBook
XSD plus adds a few of its own.
