I can't speak for anyone else, but I have many times adapted DocBook (and TEI, for that matter) to handle details they were never designed for. This is because the underlying structure â which does not usually need changing â is both robust and very common; and because the creation of RNG customisation layers or DTD shims is not difficult.

In most cases it means adding new attributes and occasionally changing a content model slightly. It almost invariably means hijacking some otherwise unused element types for purposes specific to the knowledge domain (sometimes called "tag abuse"). But provided you document the changes so that others can see what you have done, I think it is the most effective approach.
