I made the changes for this issue. I think it's fixed with
the schema and doc changes. THere is the minor issue
that Marty brought up in his analysis.

201:  Duplicate sections for attributes/elements that reoccur

Most of the elements/attributes when they are resused have a different
context to them and hence are being described again. In that sense
I think the reoccurence wherever it is happening is ok
