New erratum in CPPA version 1

The table of contents of CPPA version 2 contains two extraneous lines, 
lines 142 and 156 in the PDF.  Each line contains a non-normative note 
about ebXMLSenderBinding or ebXMLReceiverBinding. I checked the places in 
the Word document that these two lines refer to.  In each case, in the 
text, the line in question has the head-3 style whereas it should have the 
Normal  text style.
