Question about the digest in Receipt

Question about the digest in Receipt:

I assume the digest is to cover all payloads either in SOAP Body, or in attachments.

Our implementers tell us there may be a performance issue when creating the digest for large payloads at the [user message] Receiver side. E.g. the stream may be read once-only (can't be repeated). Do we assume that all payloads have to be saved by the receiving MSH prior to transfer to consumer app?
