Subject: T2 Some Additional Editorial Fixes

David:

Here are some additional (previously unreported) editorial fixes I like to
suggest:

- Change reference messageHeaderv0_99.xsd to point to the proper version
(example on lines 601 to 617).

- Clarify if the rule on line 894 "A MSH that receives a message with a
sequenceNumber element MUST NOT pass the message to an application as
long as the storage required to save out-of-sequence messages is within
the implementation defined limits and until all the messages with lower
SequenceNumber have been received and passed to the application." applies
to intermediate MSHs. Note: Figure 8-2 (line 1061) shows a Routing
application that sits on top of an intermediate MSH.

- Change PartyA-id to PartyB-id on line 1099 to be consistent with the
caption "Transmission 2 - Message X From Party B to Party C". Also
change PartyB.com/PartyAMsh on line 1029 to PartyB.com/PartyBMsh and
  change PartyC.com/partyBMsh on line 1133 to PartyC.com/PartyCMsh.

- Change "vi a the network connection" to "via the network connection on
  line 2106".

- Remove the simpleType "type.type" definition on line 2605 since it is not
referenced from anywhere in the XSD.

- Remove the clause (form="unqualified") from line 2648 since this is a
top-level attribute (and not a locally declared attribute).

- Update imported schema files to make them conform to the W3C
  Recommended version of XML Schema.

- Update specification of default and fixed values for conformance with the
W3C Recommended version of XML Schema.

- Change uriReference to anyURI to conform to the W3C Recommended version
  of XML Schema.

- Change timeInstant to dateTime to conform to the W3C Recommended version
  of XML Schema.

- Delete lines 2893-2894. The statement looks out of place.
