I have updated the document with most of the comments although there were a number I could not locate. I have opted to leave Appendix B in at present as I think it is useful and that it would just get lost if it was
placed in a separate document.

In addition, I have corrected my employer’s name as our name changed with the recent election. I also fixed a minor issue with the last diagram.
