Description
Proposal to revert the Attribute Index handling back to the previous
approach (where an Attribute Index not being specified means the Attribute
Index is assumed to be 0 in all contexts).

The changes are from the review version of 1.1 only reverting the items
related to requiring attribute index to always be specified and defining an
index of -1 to have special meaning.

The error conditions for Modify and Delete were also fixed to match the
descriptions (the older descriptions were incorrect for KMIP 1.1).
