ECF 4.0 Draft Specification

Here are my proposed changes to the ECF 4.0 specification, the mapping
spreadsheet and the ArgoUML model for discussion at the Louisville
meeting Monday and Tuesday.  I've added the mappings for Appellate and
Violations since our conference call last week.
