ECF 4.0 - NIEM conformance

ran ECF 4.0 through a new tool for testing NIEM 2.0 conformance and
identified a a number of minor issues, mostly related to documentation.
I went ahead and made the necessary revisions to the schemas to remedy
these issues.  If and when we publish a bug fix update to ECF 4.0, we'll
want to incorporate these changes.
