Binary metadata example document - More tinkerings

I made some more revisions to the example. The structure now is of a
witnessed document transaction which is the root element, with two
children: the signed document and the notary document, which may be a
jurat, certificate or acknowledgement. The two are associated logically
by the metadata and signatures. Obviously there are still clumsy
constructions and structures that can be made more elegant. Your
evaluation and help is requested.
