Updates requested at face to face

1. Separate maxima for Client and Server.  Client sets the maximum that it will accept on CONNECT, Server sets its maximum on CONNACK

2. Maximum will be the max alias value that can be used, not the total number than can be used. Max of 10 means 1..10  are valid. Max value of 0 means no aliases are allowed. 

3. Topic aliases are accepted, regardless of whether the message itself was accepted

4. NACKS will be left for now.  Future JIRA might remove them and replace with disconnection
