So we need to know how far back in time we need to get the participants list. I extracted the roster and took the participants list starting June/July last year when the official TC for v5 was formed. Then from that list extracted the list of participants who joined more than zero meetings. If we need to go back in time we just need to know till when and that member list could be extracted. 

So the suggestion is to use

Givenname Surname format 
Sort by Givenname ? 
Is that the consensus for Appendix A ?
