        Small improvements
 - Rule: names can now use same spelling as in ABNF
 - FailAt: 0 now works, needed for input that fails to parse at the first character
 - Rule coverage calculated if all test cases succeed

compile ABNF into parser and run parser on a set of test cases defined in
an XML file
