Ed. Note: An explanation how index entries are assigned to the levels has been added. It needs to be checked.

Ed. Note I haven't sorted this one out, at least not completely. We appear to be using level in the sense of numbering, since the value is positiveInteger, but also in the sense that the number implies a level, something we have yet to define. 



Patrick: Poorly worded editorial note. Say that I specify level "3." Given a particular style, which includes an outline definition, how does that value specify some part of that outline definition? It is a referencing issue.
