What does "focus" mean in this context? (I do know what it usually means that a control has keyboard focus in an interface, but since we are describing a file format I fail to see how this could be the meaning here.)

In any case, even if it means "keyboard focus", then the statement is quite meaningless:  It does not say what happens immediately following the click. If an implementation decides to immediately move the focus to another element, then it still has satisfied the current statememt. So a user may noever be able to tell that for the instance in time coinciding with the click the focus is on the element.
