In the new first sentence of 5.5.1, where you propose

The <text:tracked-changes> element acts as a container for <text:changed-region> elements ...

It is better to remove "acts as."  I recommend

The <text:tracked-changes> element is the parent of the <text:changed-region> elements ...

I have confirmed that the statement of scopes is accurate and there is no situation where we have to deal with one scope inside another.  Great!

The "not possible" statement in the new second paragraph needs work.

Replace the new text 
Change tracking in a certain scope of an OpenDocument document is not possible, if the <text:tracked-changes> element is absent in this scope.   In this case, all change mark elements [reference to 5.5.7] in this scope shall be ignored." 



If a <text:tracked-changes> element is absent, there are no tracked changes in the corresponding scope.  In this case, all change-mark elements (5.5.7) in that scope shall be ignored.

(I use "change-mark" because that is the notation in the schema.  I see that it is not hyphenated in 5.5.7 though.)
