The first page is the first one on which the page style is applied, regardless of any numbering offsets... there may be multiple first pages per page style if a different page style is applied in between 2 applications of the one with the first-page, but there is only one first page per application of a page style.

As to what happens with master documents... IIRC the child documents become sections in the master document, so they don't necessarily start on new pages or create page breaks, so i don't think there is a particular interaction to be handled here, it's basically just one big document and if there's a page break with a page style specifying a first-page somewhere it gets applied as usual.
