Michael, with your description, that "first" does neither mean a physical first nor a numbering 1 page, there is no longer a problem with sub-documents of a master-document. In case a sub-document starts with a paragraph or table which references a page style, this does not change, when the sub-document is included into the master-document. And therefore the sub-document will still start with the" first page" header/footer.
 Your description of "first page" makes the feature much clearer :) It needs to go into the description for the new <style:header-first> and <style:footer-first> elements.
