Further places are:

D1, third sentence: alt text on the <img> elements elements {color:#de350b}shall{color} be mapped

D2, first sentence: The <svg:title> element {color:#de350b}shall{color} take precedence

D4, first sentence: alt text of hyperlinks, {color:#de350b}shall{color} be mapped to

The document [http://docs.oasis-open.org/office/office-accessibility/v1.0/cs01/ODF_Accessibility_Guidelines-v1.0.odt] does not use the word "shall". So I think, we should neither use it in Appendix D. [BTW, the linked document refers to ODF 1.1.]
