The first issue could be solved in one of 2 ways: either define a maximum list level of 10, and refer to levels as %0 .. %9 - or use some sort of bracketing like %1% to ensure it's unambiguous.

Probably 19.803 text:display-levels should be ignored in case a num-format exists because that specifies the list levels itself.
19.804 text:display-outline-level is on <text:sequence-decl> - is that relevant?

The RTL question is tricky if you look at some examples - but i suspect it's actually simple: the text is generated just like for LTR, and then it's displayed according to its direction, which may change internally.
For example, "1" numbering can be RTL but  "A" or "a" numbering will switch it to LTR, possibly including the following separator chars. But probably it's just displayed like any other chunk of mixed-direction text.

For example: "%1:%2;%3." displayed as "A;a.:2"   where %1->2 %2->A %3->a
