Expense sheet application scenario

Following Andreas's suggestions, I've uploaded a new version of the expense sheet application scenario.

I've added a form mailto: action. I know that isn't very reliable, but it allows for some primitive testing. 

The javascript functions (for counting the checkboxes) may get blocked if you browse the page on a local filesystem.
