How about require that the value is an xsd:duration?  That then aligns with ISO 8601 time durations.   

If you wanted to make it totally unambiguous you could say that the elapsed time is accumulated at the finest grain of resolution, say milliseconds, and that the duration expressed is as if that accumulated time was extended from the creation time of the document.

Not necessarily more useful, but makes it unambiguous.
