Based on my experience, I think the odd statements are reflective of how heading rows tend to be identified in current applications. Establishment of headings using the software is by stating a row range and has nothing to do with the groupings, which are specified in another way.  The restrictions in ODF seem to be derived from that method of specification and its limitations with regard to adjacency of heading rows.  Of course, I established headings using the markup and found many interesting things about what happens then.
