Working forward from the contributed v2.30 files
Applying noted changes at https://wiki.oasis-open.org/pkcs11/Definitions
Correcting noted issues with v2.30
Removing historical comments no longer relevant
Removing CK_DEFINE_FUNCTION which is not used (and never has been) in the actual header files
