Fixed the P10 cert request page (was RE: [saml-dev] Web access to theSAML Demo CA)

How embarrassing. Normally I use Netscape as my default browser, just to
catch these sorts of mistakes. This time, I didn't have it installed on the
laptop so it hadn't been tested.

Fortunately, it was a "click one button and restart the web server" problem,
so it's fixed now.
