Updated test assertions document

Folks,

I've built an updated Test Assertions
document, which is loaded here:

This contains:

- the OASIS frontmatter and format
- removed the test assertions for normative
statements no longer in Assembly CD03
- added the missing test assertions
identified by Eric
- fixed the typos identified by Eric
- fixed the ordering of test assertions
