New Assembly Spec revision: CD-01 Rev4

I have just posted a new Assembly spec
revision - CD-01 Rev4:

This incorporates:
- issue resolutions for issues 32 &
96
- editorial fixes relating to application
of Issue 57
- editorial addition relating to no
operation overloading
- editorial corrections to the table
in Appendix D, where there were misplaced statements

I note that this new revision closes
the following action items:
