After a hallway discussion at the face to face, I realized that the
message ID of the first message sent to the service interface instance by
its "client" port instance, can serve as the message ID to use in the
wsa:relatesTo for all callback operation request messages to that
originating "client" port instance.

This would allow multiple callback instances to reside at the same
callback address, and be distinguished by the relatesTo header in callback
operations sent to that address.
