3. Please remove empty page 4 from the document. 

I don't see an empty Page
4, either in the PDF or the ODT formats of the document.

4. In 'Normative References', [TA-GUIDE] has a broken hyperlink. To fix,
'Edit Hyperlink' and remove the extra space at the end of the link.
