Title: Appendix describing migration from OSOA 1.0 spec for C++

Target: C++ Client and Implementation Model

Description: 
The charter for the SCA-C-CPP TC requires the inclusion of a migration
guide for porting applications written against the OSOA specification to
OASIS specification.

Proposal: 
Add an appendix "Migration from the OSOA SCA C++ Client and
Implementation Model 1.0 (non-normative)" to the C++ Client and
Implementation Model specification to maintain any migration issues.  As
changes are introduced to the C++ Client and Implementation Model
specification, notes should be added to this section describing the
change necessary to move from a 1.0 compliant model to a version
compatible with this version of the spec, as well as any implementor
notes or recommendations for how backward compatibility may be
maintained.
