Subject: Some reformatting

The assembly specification has been
reformatted in an effort to make it easier to understand. One of
the key parts of the reformatting is the way the schema elements are presented
and described. This draft does a similar reformatting for the C++
spec as well as removing some of the text that was duplicated from the
assembly spec.
