JAVA-98: Revised proposal

Here are the revised versions of the Java CI and Java CAA documents with
changes for JAVA-98, based on the discussion on Friday's call.

In the Java CAA spec, please take a close look at JCA70002 in section 8.3
and JCA70005 in section 8.5.  I have made more extensive changes to these
than the one change that we discussed on the call.
