[Issue 15] Proposal - inlined into the Policy Spec

Here is a version of the Issue 15 proposal
done inline in the specification.

It is based on the WD-07 document published
previously.  All change marking was left IN, if you want to see the
new stuff
only use the Word doc and turn off the
reviewers other than Mike Edwards:
