CD02 - revision2 available for use/review

I've just published CD02 rev2 containing application of issues 81,84,85,86,95,96,98,99. These were all the resolved but unapplied issues to date. I accepted all rev1 changes and then applied the issue resolutions.

Please use these documents as the basis for creating issue resolutions; accept all changes first and then write your proposal with change tracking on.
