Here is the latest revision with all of the updates discussed at the last
meeting including the Environment.setInstance(Environment) method.

- Changed Environment.createHelperContextFactory(ClassLoader) to be
protected static
- Added two new classes:
  commonj.sdo.impl.CreateEnvironmentException
  commonj.sdo.impl.CreateHelperContextFactoryException
- Changed instances of RuntimeException to instances of the above
exceptions
- Added Environment.setInstance(Environment) method
