ALERT! Ballot -- RE: Ballot for Issue Groups 2, 3, 4, 10, 12,and 13 attached

There is an error in the ballot. The responses for UC-4-01 and the question
for UC-4-02 were dropped in the ballot. Therefore, the responses to UC-4-02
appear immediately under the question for UC-4-01. If you use the ballot as
is, your vote for UC-4-01 will be reversed.
